I agree to have Holliston Oil Service contact me via the information I provided to set up a new customer account for services. I understand that payment information will need to be provided by me to set up a residential charge account application for ongoing services.

Authorization to bill credit card: I authorize Holliston Oil Service Inc. to automatically apply all charges for heating oil delivery, maintenance, heating equipment and heating services to my credit card on file. I understand that all charges will be paid on or about the day of service and I will receive a receipt as proof of payment via email and/or mail. I agree to the terms and conditions below. This agreement will be valid unless I notify Holliston Oil Service Inc. to discontinue use of my credit card.

I authorize Holliston Oil Service to check my credit record and to report to credit bureaus and lawful recipients my performance of the account applied for.

Terms:

All deliveries below the minimum requirement of 125 gallons for residential call accounts will be subject to a delivery fee.

All payments must be paid in full on the designated due date following the date of purchase. Net amounts due in thirty (30) days. Late fees incur upon thirty-one (31) days. Upon default, a finance charge of 1.5% will be assessed on any and all amounts past due. In the event of non-payment, the customer agrees to pay all costs associated with collection of delinquent accounts, including any attorney fees.

A service charge of $35 will be assessed for each check that is returned unpaid for any reason.

Customer agrees to inform Holliston Oil Service Inc. in the event of change in information, payment information or banking institution.

All invoices will be sent via email unless otherwise requested
